Healthy Athletes Screenings

Each athlete must have a consent form signed and submitted prior to screening. If the athlete is under the age of 18, a parent or guardian must sign giving permission for his/her athlete to attend a screening. Please ensure your athlete has the opportunity to attend a screening by signing this consent form.
